how to make an adult referral

If you’re a professional supporting an adult (18+) in Stoke-on-Trent who needs help with drug and alcohol challenges, you can refer them to CDAS by completing our form and emailing it to referrals@sotcdas.org.uk.

If the person you’re supporting would rather refer themselves for support with drugs and alcohol, they can do so by calling us on 01782 283 113 or emailing us via referrals@sotcdas.org.uk.

how to refer a young person

Under 18s 

If you’re worried about a young person in your care (under 18), you can refer them for support to CDAS. 

We have a specialist team of Young Person’s workers who can offer tailored support, at their own pace. 

To refer a young person, complete our referral form and email it to ypreferrals@sotcdas.org.uk. 

19-25 year olds

While our young person’s service is mostly catered for under 18s, we do support young people between the ages of 19-25 within this service when deemed more appropriate to their needs.

To refer a young person aged between 19-25, please complete our form and send it to ypreferrals@sotcdas.org.uk.
